NYC REACH is hosting trainings for member practices to provide an in-depth understanding of Chronic Care Management (CCM) and Transitional Care Management (TCM) services in 2019.

Attendees will learn:

  • The scope of services required to bill for CCM and TCM services

  • Documentation and coding methods for appropriate CCM and TCM reimbursement codes

  • How to identify patients that require complex chronic care services

  • Best practices and workflow implementation methods for CCM in a value-based care model


Please note, the same training is offered on two dates at different locations.

The Friday, May 17, 2019 from 9:00 AM - 1:00 PM training will be held at 80 Centre Street, New York, NY 10013

The Tuesday, May 21, 2019 from 9:00 AM - 1:00 PM training will be held at 42-09 28th Street, Long Island City, NY 11101
