NYC REACH is hosting trainings for member practices to provide an in-depth understanding of Chronic Care Management (CCM) and Transitional Care Management (TCM) services in 2019.
Attendees will learn:

The scope of services required to bill for CCM and TCM services

Documentation and coding methods for appropriate CCM and TCM reimbursement codes

How to identify patients that require complex chronic care services

Best practices and workflow implementation methods for CCM in a value-based care model
Please note, the same training is offered on two dates at different locations.
The Friday, May 17, 2019 from 9:00 AM - 1:00 PM training will be held at 80 Centre Street, New York, NY 10013
The Tuesday, May 21, 2019 from 9:00 AM - 1:00 PM training will be held at 42-09 28th Street, Long Island City, NY 11101
